Protection System misoperations show up on the NERC Reliability Coordinator (RC) exam because a single relay operating incorrectly can turn a normal fault into a cascading event. NERC Reliability Standard PRC-004 is the standard that governs how misoperations are identified, reported, and corrected. Expect exam questions on who owns the misoperation, what counts as a misoperation, and what the RC's role is when one occurs.

  • Know the applicable entities. PRC-004 primarily applies to the Transmission Owner (TO), Generator Owner (GO), and Distribution Provider (DP) that owns the Protection System — NOT the RC directly. The RC's role is coordination and awareness, not ownership of the corrective action plan. Distractor answers will try to put the RC on the hook for repairs.
  • Memorize the three misoperation categories. NERC defines a misoperation as (1) a failure to operate when required, (2) an unintended operation (operating when not required), or (3) a slow trip for in-zone faults. If a question describes a relay operating for an out-of-zone fault, that's an unintended operation.
  • "BES Element only" — PRC-004 applies to Protection Systems that protect Bulk Electric System (BES) Elements and to Composite Protection Systems whose operation or failure to operate affects the BES. Sub-BES equipment issues are a distractor.
  • RC coordination angle. When a misoperation contributes to a disturbance, the RC is typically the entity coordinating the wide-area picture (per IRO-001 authority and IRO-008 assessments). Look for answers that keep the corrective action plan with the asset owner but keep the RC informed for situational awareness.
  • Do not confuse PRC-004 with PRC-005. PRC-005 is maintenance of the Protection System (intervals, testing). PRC-004 is what to do after a Protection System operation is judged to be a misoperation. A common trap is asking about maintenance intervals under a PRC-004 stem.
  • Corrective Action Plan (CAP). If the entity identifies a misoperation, they develop and implement a Corrective Action Plan. Exam answers that say "no action required" or "only report it" are wrong when a misoperation is confirmed.
